Published in Nutrients, the 12-month study by researchers at Tianjin Medical University in China found that co-supplementation suppressed blood biomarkers of neuroinflammation and delivered superior improvements in cognitive scores.
“These findings highlight the potential of this combined supplementation as a safe, accessible, and mechanism-driven nutritional strategy for early intervention in cognitive decline,” the researchers wrote.
Dual neuroprotective mechanisms
The trial builds on existing literature surrounding the independent brain health benefits of both vitamin D and omega-3 fatty acids.
Vitamin D3 binds to its receptors in the brain to inhibit neuroinflammation and reduce amyloid beta accumulation. However, optimal blood levels for slowing the progression of age-related cognitive decline remain undefined in current clinical guidelines.
In previous studies, omega-3 fatty acids, particularly DHA, have been associated with lowering the risk of cognitive decline and dementia by up to 20%. As the most abundant omega-3 fatty acid in the brain, DHA is essential for maintaining neuronal membrane fluidity, supporting nervous system communication, and stimulating neuroprotective factors such as brain-derived neurotrophic factor (BDNF).
Previous evidence suggests DHA may modulate vitamin D receptor activation, indicating that their mechanisms may be complementary and that their combination could be superior to single nutrients in preventing cognitive decline.
The category’s potential for combining the two nutrients is substantial, driven by an aging global population and consumer interest in preventive health products.
Study details
The study involved 355 participants aged over 65 with MCI who were randomly assigned to four groups that took a daily supplement for 12 months: VD3 (800 IU/day), DHA (1 g/day), VD3+DHA (VD3 800 IU/day + DHA 1 g/day), and placebo.
At 6- and 12-month time points, the researchers assessed cognitive function using appropriate scales.
Additionally, to explore underlying mechanisms, they measured a panel of five key blood biomarkers associated with NLRP3-mediated neuroinflammation, an inflammatory process in the brain present in neurodegenerative disorders.
The researchers observed that at 12 months, all the intervention groups showed significant improvements in cognitive function and reductions in the five blood markers compared to the placebo group.
When compared to the single interventions, the combined VD3 and DHA showed “superior cognitive improvements” in cognitive scores and several subtests and reduced markers for
pyroptosis, an inflammation-driven type of programmed cell death. The combined supplement also significantly reduced other inflammatory markers in the blood panel compared to the single supplements.
The researchers called for further large-scale, randomized, double-blind, placebo-controlled trials with extended follow-up to validate these findings.
